We are a nurse practitioner–led practice specializing in evidence-based obesity medicine, mental health care, and hormone replacement therapy (HRT). Our mission is to empower you to take control of your health through personalized treatment plans, compassionate support, and a collaborative approach that respects your unique journey.
We offer telehealth services for weight management and HRT in multiple states, along with insurance-based mental health care for Nebraska residents. Our care model is built around transparency, education, and a strong patient-provider partnership so you always understand your treatment options and play an active role in your health decisions.

Our Providers
I’m Kim Wohlwend, MSN, APRN, a dual ANCC board-certified Family and Psychiatric–Mental Health Nurse Practitioner. I provide evidence-based care that connects weight and metabolic health, hormone therapy, and mental wellness, because improving one supports the others.
Through multi-state telehealth, I help adults navigate obesity, metabolic conditions, and hormone imbalance, with a safety-first, education-focused, and collaborative approach. My care philosophy is straightforward: listen first, use the best available evidence, and partner with you to build long-term results rather than quick fixes.
I hold licensure in 16 states, including Nebraska, Iowa, Vermont, New Hampshire, Idaho, Kansas, South Dakota, North Dakota, Utah, Colorado, Kentucky, Montana, Arizona, Maine, Illinois, and New Mexico, allowing high-quality care to be accessible wherever patients are located.
For patients in Nebraska, I provide compassionate mental health care for anxiety, depression, ADHD, and mood-related changes, including support during perimenopausal or postpartum transitions. Treatment may include medication management combined with practical skills, lifestyle strategies, and shared decision-making.
In addition to clinical practice, I serve as Nebraska’s faculty liaison adjunct for APRN students, supporting the next generation of nurse practitioners and advocating for patient-centered care.
My career has included digital medicine, house-call practice, retail clinic management, and inpatient care at Northwestern Memorial Hospital in Chicago, experiences that shaped my commitment to communication, compassion, and patient safety.
I earned my MSN–FNP from Clarkson College (Summa Cum Laude) and completed graduate training as a Psychiatric–Mental Health Nurse Practitioner, reflecting my belief that mental and physical health are inseparable and should be treated in connection with one another.
My approach remains consistent: listen first, apply the latest evidence, and partner with you for sustainable mind-and-body wellness.
