Services We Provide at No Cost
For People with Disabilities
We provide appropriate auxiliary aids and services, free of charge, to help you communicate effectively with us. These may include:
- Qualified sign-language interpreters (in person or by video remote interpreting) for scheduled visits.
- Real-time captioning for video visits.
- Written materials in large print, accessible electronic formats, or other alternative formats.
- Extended appointment time when needed for effective communication.
- Assistance with our website and patient portal. See our Accessibility Statement.
For People Whose Primary Language Is Not English
We provide language-assistance services, free of charge, so you can communicate with us in the language you understand best. These may include:
- Qualified medical interpreters (in person, by phone, or by video) in your preferred language.
- Translated written materials on request for key documents.
- Bilingual staff or bilingual telehealth options where available.
To request any of these services, please tell us when you schedule, or call us at 531-217-5257. Advance notice helps us arrange interpretation, but we will do our best to accommodate last-minute requests.
If You Believe We Have Discriminated
If you believe that Midwest Mind & Body Healthcare has failed to provide any of the services above or has discriminated against you on the basis of race, color, national origin, age, disability, or sex, you may file a grievance with us and separately with the U.S. Department of Health and Human Services.

You may submit a grievance in person, by mail, by phone, or by email. If you need help filing a grievance, the Section 1557 Coordinator is available to assist you. We will investigate, respond in writing, and provide you with our decision and the reasons for it. We will not retaliate against you for filing a grievance.
